New patients are
always welcomed!
Our staff will be pleased to
assist you in any way possible and all efforts will be made to accommodate
your schedule.
Everyone at our office is at your service.
Cancellations
Patients are seen by
appointment only. We make every effort to be on time for your
appointment and ask that you extend the same courtesy to us. If an
appointment cannot be kept, please give our office a 48 hour (2 business
days) advance notice. We reserve the right to charge any patients that
chronically fail to keep their appointments.
Emergencies
Our office provides on call
service for emergencies, 24 hours-a-day, 7 days-a-week for all patients
of record. You may page Dr. Clary or Dr. Fueredi with an emergency
by calling (239) 262-9320.
Telephone
Calls to the Dentists
If you have medical dental questions which only the dentist can answer, a message will be taken and
your medical dental file will be given to Dr. Clary or Dr. Fueredi who will
return your call the same day. If your question is non-medical, a
qualified staff member will be pleased to assist you.
Emergency
Care for New Patients
All emergency care patients
are expected to pay 100% for their treatment at the end of their
appointment. If you have dental insurance, we will submit this to
your insurance company and we will have the payment sent to you.
Patients with
Dental Insurance

Your
first appointment with us is payable by you. In the future,
after preauthorization on, all deductibles and co-payments must be
paid at time of service.
Note:
If we are unable to confirm patient
eligibility and complete insurance information, the full amount is
payable by you.

All
prosthetic restorations must be pre-authorized by your insurance
carrier. Once pre-authorized is filed, a final payment may
come from your carrier. Adjustments will be made if any
overpayment occurs.

Finance
charges will accrue at 1.5% per month $2.50 minimum, on balances
over 60 days old.
Non-Insured
Patients

Payment
for treatment is due at time of service. We accept cash,
checks, Visa and MasterCard.

For
prosthetic services, 1/2 is required at start of treatment, and the
balance over the remaining appointments. Full payment is
expected prior to completion of the case. Extensive cases may
require special arrangements. A payment schedule will be
established for you at the treatment conference.

Finance
charges will accrue at 1.5% per month, $2.50 minimum, on balances
over 30 days old.
